Terms and Conditions

General

  • An appointment at Fysio.Center can be made via e-mail, telephone or verbally. See herefor the contact details

  • For a first treatment you can report to the desk of the yoga studio "Bluebirds" and then take a seat in the waiting room. You will be picked up from the waiting room by the therapist.

  • You do not necessarily have to report to the desk for the following appointments. It may be that the therapist is running out. If you wait longer than 10 minutes for the therapist, come and knock on the door of the treatment room.

  • Fysio.Center has consciously chosen not to enter into contracts with health insurers. This means that the treatments are not declared to the health insurers but that you have to pay for the treatment yourself. You can then submit the invoice to your health care provider. The treatment is largely, and sometimes entirely, reimbursed by the health insurer.

  • Not all health insurance companies reimburse DTF (Direct Accessibility Physiotherapy). This means that you must first obtain a referral from your doctor before you can come to Total Care Fysio. Check with your health care provider whether DTF is reimbursed. If this is not the case, you should get a referral from your doctor.

  • At the end of the treatment you can pay by debit card or contact, after which you will receive the invoice by e-mail.

  • You must be able to identify yourself at the intake.

Cancel an appointment

  • If you want to cancel or move an appointment, you must do so at least 24 hours in advance. If you do not do this, an amount of € 25.00 will be charged. If you are late for an appointment, please contact us by telephone.

Treatment process

  • During the first treatment, an intake takes place in the form of a case history. This is followed by a physical examination.

  • During the physical examination you will have to remove certain items of clothing so that the body can be examined better. If you have difficulty with this, you must report this to the therapist. Then another solution is sought. If you do not like to stand in your underwear, you can put on small pants.

  • The transfer to third parties will take place by means of a written notification, in which the personal details (including BSN, personal details, insurance details) and the treatment process are stated and described. This transfer is made in accordance with and with the approval of the patient / client.

  • In the event of a different course in duration or in symptoms, it may be decided to refer the patient / client to the doctor for any further medical examination. For this purpose the transfer will take place, as described in the point above.

  • At the end of the treatment process, this is concluded with a final report to the referrer.

  • The reports are sent by mail, via a secure internet connection or by letter which is given to the patient.

House rules

  • You must bring your personal items to the treatment room. Fysio.Center is not liable for loss, loss, theft, or damage to property of patients, clients, or others who enter the practice.

  • The use of mobile phones during treatment is not appreciated.

  • In the event of an emergency you must follow the instructions of the personnel or the competent authority (police, fire brigade).

Privacy and handling of patient data

  • All data and information will be treated confidentially in accordance with the Data Protection Act. There will be no exchange with third parties without this in consultation and agreement with the patient.

Complaints procedure

  • If you nevertheless have a complaint about how you have been treated, about the course of the treatment process or procedure during treatment, it will be greatly appreciated if you report this to your therapist. In this way we want to keep the quality of care as optimal as possible, and hope to learn from our mistakes.

  • If you have a complaint that you do not want to report within Total Care Fysio, you can use the official complaints procedure of the KNGF. Information about this can be found on the KNGF consumer site (Fysionet.nl), where you can download the brochure "A complaint about your physical therapist" directly. Here you can find the procedure of the complaints procedure and where you can go with your complaint.

Payments

  • Fysio.Center has consciously chosen not to enter into contracts with health insurers. This means that the treatments are not declared to the health insurers but that you have to pay for the treatment yourself. You can then submit the invoice to your health care provider. The treatment is largely, and sometimes entirely, reimbursed by the health insurer.

  • The patient is responsible for the payment of the invoice. Check with your health insurer what you will be reimbursed per treatment. Ask for the reimbursement of physiotherapy.

  • If you cannot meet your financial obligations, it is possible to make a payment arrangement with Fysio.Center. This will be done in consultation with the therapist being treated. The therapist and patient will come to an agreement together.
